Outreach

Outreach for clients takes place in bars/clubs, Laundromats, business establishments, neighborhood centers, low-income housing complexes, social organization, street locations, parks and beaches, homeless/domestic violence shelters, jails, sexual assault treatment centers, soup kitchens, substance abuse treatment center and mental health families etc.   Program staff collaborates with existing outreach programs and establish reciprocal referral relationships with other outreach workers.   They also establish specific, communal referral processes with existing provider networks establish relationships with community gatekeepers, develop and implement strategies to ensure program success in Broward County.   Our agency consistently maintains qualified staff to provide an effective and successful system of care designed to serve vulnerable populations within our county.   In fulfilling these responsibilities, CHFS staff participates in capacity training, media activities targeting Human services and participate in relevant community collaborative initiatives.

 

Comprehensive Service Assessment

A thorough, comprehensive service assessment is the foundation upon which to build the service and/or treatment plan for a client and/or family.  CHFS conducts initial assessments of varied clients to offer them an outstanding service delivery while taking into consideration cultural sensitivity and assimilation especially if they are from immigrant families.  The assessment itself shall be based upon the strengths and needs of the client and /or family and shall include their input.   The client and/or family shall provide the information under the guidance and questioning of the interviewer and shall participate in the development of goals and in the selection of services to be incorporated into the client service plan and /or treatment plan.

 The assessment shall focus on the strengths of the client and/or family as much as possible.  Strengths are frequently overlooked as important factors in the process of change when, in fact, they are the key to making successful changes.  A strength based assessment should provide a positive vehicle for engaging clients and /or families, for promoting creativity and innovation, and/or forming the basis of goal selection and reinforcement.

 

Services

We offer prevention, education and early intervention services in the areas of teenage pregnancy, school performance, adolescent development, juvenile delinquency, family and dating violence, child neglect, job placement and financial literacy.   Critical to the success of the agency’s mission is its emphasis on health promotion, skill building, personal and family resiliency.

We also provide services in the arena of hunger, transportation, housing, immigration, translation and interpretation, health, workforce development, technology, refugee related issues, senior support, prosperity, family health and medical insurance, utility needs, foreclosure prevention, youth development, out of school time initiatives, nurses & family relationships, Income Tax Preparation, Veteran Support.
